Description

According to the Agency for Healthcare Research and Quality (AHRQ), Medication Reconciliation refers to the process of avoiding inadvertent inconsistencies across transitions in care by reviewing the patient's complete medication regimen at the time of admission, transfer, and discharge and comparing it with the regimen being considered for the new setting of care. This presentation will provide education on steps to ensure a complete and thorough medication reconciliation process for clinicians in any health care setting by incorporating everyday practice considerations including social determinants of health concerns which are associated with common adverse drug events. The material will include quizzes and case studies in post-acute long-term care as well as ambulatory care transitions one may encounter in alternate living settings.
