Description This session will review best practices in early interventions for patients and residents with chronic disease and in need of palliative and supportive care. Dr. Grossman will utilize national and local data as well as tracking and trending to discuss industry hot topics and clinical outcomes such rehospitalization reduction, polypharmacia, goals of care, family support and many more interventions designed to align goals and family, envelop residents in care, and keep them out of the hospital and comfortable at home or wherever they call home.
